**Home Care Aide (Caregiver)**
**Job Description**
The prospective applicant should be able to perform the following tasks:
Bathing-Grooming-Dressing
Toileting and incontinence care
Medication reminders
Meal Preparation
Visits to Doctors, Dentists
Light Housekeeping
Laundry and linen change for Client
Provide light exercise, like walking
Companionship
Recreation (Movies & Theaters)
Family Comfort
Visit to Friends/Relatives
Other tasks that may be assigned from time to time by the Client
**LIST OF REQUIREMENTS**
Personnel Record (HCS 501)
A signed statement regarding their criminal record history. Criminal Record Statement (LIC 508)
A signed Statement Acknowledging the Requirement to Report Suspected Abuse (SOC 341A)
Training verification log and documentation of successful completion of training (HCS 500)
5-hour Caregiving Training (should be done every year)
Latest TB Clearance
Form W-4 for 2021
I-9 Form
Employee Medical History Record
Employee Health Examination
Latest Covid-19 test results/Vaccination Card
A copy of CA Drivers License or ID
Home Care Aide (HCA) Per ID#
