myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. About this Role

This is an opportunity for a mission-driven team player who has experience managing the administration of both home care and home health services for PACE program participants, including the coordination of community-based care. You are a collaborative team member who works as part of the Care at Home team to help participants meet their goals. This is primarily a center-based (onsite) role. 

Expectations for this Role

• Coordinates ongoing home care and home health services for participants with external vendors 

• Schedules all appointments for home care and home health into EMR 

• Acts as vendor’s main point of contact for home care and home health needs 

• Ensures home care and home health medical records are received for participants in timely manner 

• Completes appointment reminders for home care and home health appointments 

• Collaborates with Home Care Coordinator and other interdisciplinary team members when coordinating participant care 

• Collaborates with the Transportation Coordinator to ensure accuracy for participant transportation 

• Passion and mission orientation for serving high-risk seniors and frail older adults 
• CPR and First Aid certification (within 60 days of employment, we will provide) 
• Experience with frail and elderly adults 
• Home Care and Home Health coordination experience

• This role is expected to be onsite 5 days/week
