CareAparent is seeking an experienced Physical Therapist (PT) for a part-time opportunity performing home health visits. This position is for a PT to provide services to geriatric clients throughout the Saint Cloud area. Prior home care experience preferred, but not required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary that provides rehabilitative care to clients, allowing them to live independently and age in place safely.

RESPONSIBILITIES:

• Evaluate and treat clients suffering from physical disability due to injury, disease, or surgery in a home care environment
• Provide treatment for clients using a variety of methods including exercise, hands-on therapy, stretching maneuvers, and equipment to ease clients' pain and help them increase their mobility
• Test, measure, and assess clients' current condition and abilities
• Assist patients with all phases of Physical Therapy treatment and techniques to include exercise instruction, manual therapy techniques, activities of daily living, functional/total body progressions, modalities, and a fall prevention program
• Establish treatment plans specific to the client's specific needs that allows for excellent client outcomes
• Make recommendations for devices or tools that might improve the client's quality of life
• Work cooperatively and consistently with physicians, case managers, nurses, occupational therapists, and caregivers
• Provide training to team members as needed
• Request interventions to help avoid unnecessary trips to the emergency room or readmissions
Monitor, document, and report clients' progress

REQUIREMENTS:

• Current Minnesota Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from an accredited physical therapy program as indicated by school transcript or diploma.
• Home care experience preferred
• Ability to work independently and manage time effectively.
• Strong interpersonal skills, organizational skills, and problem-solving skills.
• Solid computer skills: prior experience with electronic medical records (EMR) preferred.
