Due to rapid growth, Care IV Home Health is now hiring a Physical Therapist for PRN or part-time home health physical therapy visits.

Physical Therapist Duties may include, but are not necessarily limited to:
- Visit patients in the comfort of their homes and assisted living facilities
- Develop & follow physical therapy care plans, performing PT, documenting / charting patient progress, patient admits / discharges, OASIS assessments, updating Therapy Director on patient status, etc.
- Other duties as assigned by our Director
Physical Therapist Desired experience:
- Physical Therapist actively licensed or registered in the state of Arkansas
- Minimum 6 months of PT experience required
- Home Health experience a plus, but not required
- As a healthcare provider in compliance with CMS guidelines & conditions of participation, we require a COVID vaccine as a condition of employment (absent reasonable accommodations)
