Position: Case Manager

Location: Queens, NY

Schedule: Monday to Friday, 9:00AM – 5:30 PM

Hourly Rate: $18

Job Description:

Are you looking for a job where you can make a positive impact on the community? If so, a shelter for underserved NYC residents is searching for a Case Manager in the Queens area! You will be responsible for all aspects of the assigned case load, including meeting with clients on a regular basis and ensuring that all clinical files and reports are complete and up to date.

• Committed to engaging individuals in a client-centered approach while providing services and referrals to address the varied needs of individuals experiencing homelessness, including securing entitlement, gainful employment, and housing.
• Have an ability to manage conflict or challenging behaviors with self-awareness and emphasizing client autonomy and safety.
• Help to build clients' daily living skills to ensure successful independent living.
• Responsible for completing intakes and assessments and all required documentation, including psychosocial, progress notes and treatment plans.
• Escort clients to various appointments, as needed.
• Participate in interdisciplinary team meetings and case conferences.
• Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

Job Type: Full-time

Pay: $17.00 - $18.00 per hour
