We are seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will play a crucial role in providing support and assistance to individuals and families in need. Responsibilities:
- Conduct comprehensive assessments of clients' social, emotional, and psychological needs
- Develop individualized treatment plans based on assessment findings
- Provide counseling and therapy services to individuals, families, and groups
- Collaborate with healthcare professionals to coordinate care and ensure holistic treatment
- Advocate for clients' rights and access to resources
- Assist clients in accessing community services and support programs
- Monitor client progress and adjust treatment plans as needed
- Maintain accurate and up-to-date client records
Experience:
- Bachelor's or Master's degree in Social Work or related field
- Previous experience in crisis management, clinical counseling, or behavioral health preferred
- Knowledge of ICD-9 coding and medical terminology
- Familiarity with applied behavior analysis techniques
- Understanding of hospice care principles and practices
- Proficiency in HIPAA regulations and maintaining client confidentiality
