Halcyon Home, a trusted and locally-owned home health and hospice company in Austin, is currently seeking a dedicated and experienced Hospice Social Worker to join our compassionate team. This is a full-time position based in our Austin office, with a rotating on-call weekend once every 5-7 weeks. The ideal candidate must be licensed as an LMSW, have at least one year of hospice care experience, and possess a deep passion for working with the elderly. Responsibilities:

• Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients
• Conduct comprehensive assessments of patients' social and emotional needs
• Develop personalized care plans in collaboration with the interdisciplinary team
• Provide counseling and support to patients and families facing end-of-life challenges
• Assist with coordination of care and access to community resources
• Maintain detailed and accurate documentation of patient interactions and progress
• Utilize Kinnser software (knowledge of which is a plus)

Requirements

Qualifications:

• Licensed as an LMSW
• Minimum of one year of hospice care experience
• Minimum of one year of social work experience
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to working with the elderly
• Ability to manage a fast-paced, independent work schedule
• Knowledge of Kinnser software is a plus

Assessment: $75:00 

Routine Visit: $50.00 

Extended Visit: $60.00
