From the moment a doctor prescribes follow-on care in a post-acute environment, individuals at hospitals and skilled nursing communities work diligently behind the scenes to ensure a smooth transition for each patient and their family. We complete most admissions paperwork before an individual arrives at our door.

Bridges Health seeks a hard-working, self-motivated, and customer-friendly admissions coordinator professional. The Central Intake Admissions Coordinator works with our communities and care teams throughout Oklahoma to achieve a successful resident admission.

Preferred Experience, Skills, and Qualifications

  • Prior customer service experience and administrative skills specific to the skilled nursing and long-term care environment are preferred. We will train the right candidate.
  • Knowledge of Medicaid, Medicare, and third-party managed care payer sources is preferred. Training available.
  • Proven organizational skills accentuated by heightened attention to detail and the ability to manage multiple tasks and admissions simultaneously
  • Strong verbal and written communication skills
  • A team player mentality coupled with the ability to establish effective working relationships across the board with internal team members and external partners
  • Self-motivated, highly accountable, goal-oriented
  • Microsoft Office proficiency required

Responsibilities (include but are not limited to)

  • Direct and complete each stage of the admissions process for all assigned patients
  • Take calls.
  • Participate in the weekend on-call rotation.
  • Coordinate activity and regularly communicate with the Hospital Liaison Team and other referral sources, ensuring a smooth hospital discharge and post-acute admissions experience.
  • Other duties as assigned

Requirements

  • Able to work a flexible schedule and a rotating weekend shift
  • Successful completion of all training timelines and requirements
  • Professional demeanor and excellent customer relations skills
  • Ability to pass a criminal background check
  • Able to complete state-required fingerprinting before your start date
