The leading provider of healthcare services and senior living in Connecticut is searching for a Hospice Clinical Manager to oversee its Home Hospice Care Team. The hospice care team is essential in providing meaningful care and support to terminally ill patients and their families. In this role, you will oversee the management of patient care, hospice staff members, and patient assessments. Responsibilities:

• Directly manage a team of 10 FTEs.
• Oversee the management of patient care, coordinating all services, and acting as a liaison between all providers.
• Manage hospice staff members, including recruitment, hiring, training, and development.
• Oversee patient assessments and ensure patients are receiving the appropriate level of care.
• Consult with caregivers, insurance agencies, healthcare facilities, and other community resources.
• Educate members on the benefits of palliative and hospice care in a home setting.

Qualifications:

• A BSN is required.
• Active RN License.
• Hospice experience is required.
• 1 years of management experience is required.

Additional info:

• 40 hours/week, days
• $106,000-$112,000, depending on experience, incentives!
