A reputable home health organization in Connecticut is hiring Behavioral Health Registered Nurses for its territories surrounding East and West Hartford. In this role, you will provide direct patient care, complete initial and ongoing assessments, plan & document care, and communicate with other healthcare providers and community resources as needed. This is an exciting opportunity for you to change patients’ lives and work in an autonomous environment.

Responsibilities:

• Work one-on-one with patients outside of a clinical setting.
• Design, implement, monitor, and evaluate patients’ care plans.
• Coordinate services and inform MDs and necessary healthcare professionals of changes in patient’s conditions.
• Provide ongoing education and support to patients and families.
• Administer medications as needed or directed.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN License
• 1 years of home health experience is preferred.

Additional info:

• Full-time, permanent position
• Schedule: Monday-Friday, 830a-5p
• Compensation: $80,000-$85,000 base incentives mileage reimbursement
