Job Summary:
A member of Community Health and Engagement, a registered nurse responsible for the provision of an organized program directed toward community education and health improvement. Utilizes nursing skills to analyze, develop, and implement community education programs based on departmental priorities. Assists with colleague education related to caring for people from different cultures and diversity issues. Works in conjunction with other members of Community Health and Engagement.
Job Responsibilities:
1. Aggregates data to support community education and preventative health programs
2. Respond to public requests for health education programs. Works with physicians, clinical staff, and community partners to staff events. Supports interventions targeted toward population health needs and program enhancements
3. Works with Human Resources and others on diversity issues related to colleagues, patients and community.
4. Coordinates and oversees various screening programs throughout the Hartford Region
5. Provides consultation and education to selected patient populations to facilitate health promotion.
6. Serves as hospital liaison to community organizations related to health education and community needs.
7. Maintains awareness of resources and regulations at the local, state and federal level.
8. Works closely with the Regional leadership team to ensure that the Hartford Region provides healthcare and community education in concert with health needs assessment.
9. Demonstrates knowledge of care for required age-specific populations necessary to carry out safe and effective performance of duties.
10. Participate in hospital functions and events that support and health and engagement. May be required to vary shifts to meet the needs of patients and programs.
11. Travel between various locations within the community to perform duties.
12. Perform other related duties as required
Qualifications
Bachelor's Degree in Nursing, High School diploma preferred
3 years of previous nursing experience
5 years of previous nursing experience in community health
Bilingual (English and Spanish)
Current RN license
BLS Certification
