Job Summary:
We are seeking a dedicated and compassionate Assistant Case Manager to join our team. As a Assistant Case Manager, you will be responsible for providing comprehensive support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients' rights and well-being. Must have at least 2 years of related work experience in the Assisted Living , Adult Home or Health Care Facility.

Responsibilities and Duties

Responsible for the coordination of services for Resident including SSA/SSP/SSI and Medicaid. Completes initial and periodic evaluations to ensure that the community continues to meet resident needs. Encourage resident participation in activities and support resident and family in adjustment to life at the community and residents' daily routine. Communicates with hospitals and skilled nursing facilities regarding resident status and discharge planning and medical professionals for appointments. Documents clearly and timely in resident electronic file.

Requirements:
- Previous experience in case management or a related field is highly desirable
- Ensure completion of initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs

- Ensure the initial case management evaluation is begun at the time of admission and completed as staff assess the needs of the resident within thirty (30) days after admission

- Ensure initial case management evaluation is kept in the resident file

- Assist in orienting new resident and family to the daily routine

- Assist each resident to adjust to life in the facility

-Assist each resident to maintain family and community ties and to develop new ones

-Encourage resident participation in facility and community activities

Pay: $27.00 - $29.00 per hour
