Summary - Quality Clinical Care Manager:

BMC is currently seeking RNs to join our team as a Quality Clinical Care Manager. This position will work full-time 40 hours per week Monday through Friday and will enjoy evenings, weekends, and major holidays off. This is an exciting opportunity for Nurses who want to step away from face to face patient care but still be very much apart of the a patient's healthcare journey. This position is also benefit eligible for the full benefit package.

Under the direct supervision of the Care Management Manager and the guidance of the Director of Nursing (DoN), the Quality Clinical Care Manager provides care management and self-management support for the patients of the primary care departments. Monitors Coumadin patients. Utilizes population and disease management registries provided to develop, implement, and evaluate individualized patient care plans, and along with Care Coordinator provides coordination of care within all applicable interdisciplinary areas of the practice. Assesses patient care, the medical treatment plan, and acts as the liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.

Job Duties include but are not limited to:

  • Coordinates healthcare services for patients through assessment of their chronic conditions and/or other health care needs.
    • Utilizes the case management process to guide service delivery throughout the healthcare continuum to ensure quality care is delivered in the most efficient and effective manner.
    • Develops personalized care plan for highest risk patients.
      • Facilitates and coordinates services to develop patient-centered, individualized, and integrated self-management plans that include outcomes and goals.
      • Evaluates the patient’s comprehension / self confidence in self-management capabilities, recognizing and reinforcing change talk and managing resistance/barriers.
      • Works closely with patients engaging families to educate them on how to follow their care plan and achieve set goals for successful outcomes.
      • Engages the patient and their caregivers in understanding and setting self-management plans in a culturally and linguistically appropriate manner.
      • Documents ICP and care coordination in case management module database (NextGen, etc.)
    • Supports the Primary Care Physician (PCP) to implement the integrated plan so it achieves the desired outcomes.
    • Comprehends transition of care and medication reconciliation.
    • Coaches, inspires and supports change. Assesses patient readiness to change, and their health literacy as well as cultural and/or social aspects that may lead to potential barriers.
  • Coordination of Primary Care Anticoagulation Clinic
    • In collaboration with providers, the BMC cardiology department, and other Care Managers, maintain empanelment of anticoagulation patients and manage their care as appropriate.
    • Conduct visits for INR testing and medication adjustment via protocols, as appropriate.
  • Performs Population Management using both registry functionality within EHR as well as reports from Clinical Quality Metrics and Payer Feedback Reports in order to identify gaps in care around evidence-based guidelines. Utilizes this information to guide the work of the Quality Improvement Team and suggests adjustments in internal workflow to improve patient health outcomes.
  • Collaborates with all members of the healthcare team to include the patient, members of the interdisciplinary care team, the patient’s family, and the physician in the clinic.
    • Collaborates with various healthcare providers across the continuum to ensure that patients are effectively managed and their healthcare needs are met.
    • Provides disease specific educational support and in-services to clinic staff.
    • Assists in the development of departmental policies and procedures, including assessment techniques to evaluate and track patient outcomes.
    • Attends and actively participates in quality and program team meetings, patient/family advisory council, webinars, and learning collaboratives.
    Required Experience:


    EDUCATION
    : Bachelor’s Degree with an active Colorado RN licensure in a clinical service area (RN) required.

    PREVIOUSEXPERIENCE: Must have a minimum of 3 – 5 years clinical experience; previous quality or care management experience preferred; cardiology or diabetes education background preferred. Must be familiar with standard concepts, practices and procedure within a particular field.

    JUDGEMENT, PLANNING, INITIATIVE REQUIRED: Must have exceptional interpersonal and communication skills to support the development of exceptional working relationships with patients, their dependents and healthcare providers; Relies on experience and judgment to plan and accomplish goals; Performs a variety of tasks that require a degree of creativity and latitude; Must have a proven commitment to customers that continually focuses on improvement and enhancement to the service delivery process.

    SKILLS/JOB KNOWLEDGE: Must possess strong analytical and computer skills with proficiency in computer and Electronic Health Record systems (EHR) such as MS Office products and NextGen; Must be able to analyze issues, predict trends and outcomes, and offer recommendations; Must be well-versed in all areas of state and federal regulations regarding HIPAA, EEO, and ADA.

    SALARY RANGE: $27.19 - $36.71/hour; depending on experience
