WHAT YOU WILL DO:
The Case Manager will be responsible for client caseload management, which may include Adult Protective Services High Risk referrals, as well as coordinating services, development of care plan and required documentation, implementation and monitoring service delivery, as well as maintaining productivity levels.
  • Provide case management services in accordance with guidelines and manual requirements as established by funding source.
  • Screen, assess and evaluate clients for service needs; and develop and implement care plans to address service needs.
  • Evaluate client eligibility for programs, including assistance with Medicaid eligibility, to when appropriate.
  • Maintain regular contact with client, as established in program guidelines.
  • Provide information, referral and follow-up for appropriate support services, including coordination of client services with Medicare or Hospice agencies, as needed.
  • Resolve service delivery problems, including immediate contact with vendors to reinstate services that have been identified as not received by client.
  • Complete and submit accurate, timely documentation, including but not limited to maintaining client record.
  • Ensure and safeguard the human and legal rights of clients, including giving client notification of rights to grieve or complain and request formal hearing or review. Will also report any suspected abuse, neglect, or exploitation to the Florida Abuse Hotline, according to State of Florida statute.
  • Maintain established productivity requirements.

WHAT YOU WILL NEED:
  • Bachelor's degree in social work, sociology, psychology, or related social services field
    • Or have a bachelor's degree in a field other than social services field and professional human services experience may substitute on a year-for-year basis for education requirement, if approved by funder as related to caseload assignment.
  • Case Manager must have minimum of two (2) years of case management experience assigned to Long-Term Managed Care Medicaid Waiver program and successfully complete all required trainings.
  • Must have a valid drivers' license and proof of auto insurance coverage.
