The Case Manager-LPN responsibilities include:

• Being responsible for patient care planning and coordination of services to patients and their family members throughout their stay and plans the discharge process for a 120-bed rehab facility with approximately 1500 patient discharges a year.
• Collaborating with physicians and other members of the multidisciplinary team, receiving, and integrating physician orders.
• Coordinating the seamless delivery of services that promotes outcomes and the highest level of patient independence and quality of life at time of discharge.

Position Details:

• Location: Morton Plant Rehab – Clearwater, Florida
• Status: Full time
• Shift: Monday-Friday 8PM – 4:30PM
• Weekend requirement: occasional
• On Call: no

Certifications and Licensures:

• Required LPN (Licensed Practical Nurse) - State of Florida

Education:

• Required LPN Diploma

Experience:

• Required 3 years in-patient nursing.
• Preferred 1 year Case Management or Patient Care Coordination
