Hope Villages of America is Hiring a Case Manager Near Clearwater, FL

Position Summary:

The Hope Villages of America Case Manager is an advocate, mentor, and life coach for the individuals in our care. The Case Manager will be responsible for working directly with individuals who have experienced domestic violence, providing crisis intervention, ensuring the completion of comprehensive assessments, individual and group therapy, comprehensive case management and advocacy services. The Case Manager will manage an individualized treatment plan for each individual and communicate with staff, community case managers, doctors, teachers, to ensure all the individual’s needs are being met. The Case Manager will schedule activities that allow for healthy social, educational, physical, and mental development. The Case Manager will build and maintain relationships with everyone, as well as community partners to ensure optimal success.

Job Specifications/Qualifications: (Min. knowledge, skills, and abilities required)

Education/Training (or equivalent): 0 H.S. Degree 1 College Degree* 0 Certification/License*

*A combination of education and experience may be considered

-Bachelor’s Degree in Social Work or related field preferred.

-Core Competency certification preferred. 

Experience (Type of work experience, min. number of years):

-2 years of experience in case management. 

-2 years of experience working with individuals in two or more of the following: mental health, related issues, complex trauma, substance abuse, court involved, domestic violence, HIV/AIDS, child welfare related issues, and crisis intervention techniques

Technical or Administrative Knowledge:

-Knowledge of Microsoft Office Productivity Tools (MS Word, Excel, outlook, PowerPoint). 

Special Skills and Abilities:

-Ability to work independently.

-Must possess a valid Florida Driver’s License and have reliable transportation.

Job Functions, Unique Duties, and Responsibilities:

Essential Functions include, but are not limited to, the following: 

General

•Follow the administrative guidelines and policies established by HVA.

•Maintain a community image consistent with the HVA mission.

•Engage in a relationship with The Haven Program Team, which is proactive, mutually beneficial, fosters teamwork, and advances the mission of HVA.

•Exercise due care and stewardship of resources which are given to HVA including equipment, food, money, volunteer and staff time.

Program Support

•Develop, maintain, and report on an individualized plan for everyone including education, life skills, independent living goals, art/music/equine therapy, as well as trauma therapy.

•Work in partnership with the program staff to promote and maintain client engagement in all programs by actively seeking out and recruiting clients and enrolling them into programs and classes.

•Work to increase client motivation to successfully move through the phases of the overall program.

•Participate in after hour on call rotation, including evenings, weekends, and holidays when necessary.

•Provide trauma informed crisis intervention when responding to afterhours calls.

•Manage emergency situations including contacting appropriate staff; interacting with police, fire, and medical personnel as needed.

•Responsible for maintaining thorough and accurate records, case files, service plans, expense reports, correspondence, monthly progress notes, and resident allowance record keeping and distribution.

Case Management

•Working from a strengths-based, trauma–informed, and culturally competent perspective, case managers will work with clients to plan, implement, and monitor service plans, safety plans, and relapse prevention plans.

•Maintain a caseload of 20 or more clients.

•Serve as linkage to services including, but not limited to, drug treatment, transitional housing, residential treatment, counseling and therapy, legal services, job training, etc.

•Assist clients in developing service plans addressing physical safety, stabilization, substance abuse, and mental health issues from a harm-reduction approach.

•Ensure immediate responsiveness for all referrals and crises for assigned caseload.

•Meet with client minimum of every week on assigned caseload and document contact in case management database.

•Continually assess clients for safety risk, protective factors, overall well-being.

•Counsel clients regarding stress reduction, goal attainment, and holistic wellbeing.

•Provide crisis intervention for clients experiencing emotional distress.

•Prepare reports by collecting, analyzing, and summarizing data results regarding methods of treatment and progress.

Advancement/Community Relations:

•Collaborate with other organizations and groups within the community to build partnerships and enhance fundraising activities.

•Assist in planning and coordinating events meant to raise funds (sanctioned events, community events, etc).

•Provide staff support at events, as needed.
