Description

Plans, organizes, and provides nursing care to residents, who reside in skilled nursing or assisted living, according to federal, state and facility regulations and standards. Ensures quality care is maintained for all residents across skilled nursing and assisted living.

Supervises Certified Nurse Aides and Certified Medication Aides.

Staff’s role in resident choices:

  • Offers residents personal control over their own lives as directed by the person-centered plan
  • Enhances residents wellness and independence whenever possible
  • Takes responsibility for personal decisions and choices that affect resident choice
  • Value and strive to honor each community member’s choices

Essential Job Functions

  • Provides professional nursing care to residents
  • Administers prescribed medications and treatments as ordered by physician
  • Takes temperature, blood pressures, dresses wounds, gives enemas, and administers other treatments as needed
  • Administers I.V.s when necessary (RNs and I.V. certified LPNs) and monitors according to accepted practice
  • Observes residents’ conditions and charts and reports changes or concerns to the appropriate parties
  • Ascertains any health issues or problems that the residents may be developing and documents according to the plan an facility polices
  • Notifies the physician, DPOA and /or family as necessary when there is a change in the resident’s condition, medication, or treatment
  • Records resident information and accidents or incidents, such as skin conditions, suspected abuse or neglect, and grievance according to federal, state and facility policies
  • Reports discrepancies in orders, care, medications, or treatments to the physician and DON
  • All nursing documentation is entered into the resident’s medical record prior to end of shift
  • Responds to residents’ calls and determines how to assist them
  • Records information regarding incident and reports to Administrator, DON, and Director of Independent Living
  • Follows accepted policies for residents’ care
  • Works with DON, physicians, Care Plan Team, nursing staff and residents’ families to assess residents’ needs, plan and modify care and implement interventions
  • Receives and records physician’s orders according to accepted practices and facility polices
  • Makes rounds with the physicians as necessary
  • Reports problems in providing resident care to the DON and recommends needed changes in facility policies and procedures
  • Participates in committee meetings and Care Plans as needed
  • Assists in admitting residents
  • Communicates necessary information to nursing staff
  • Helps residents with activities of daily living (ADLS) and ensures that residents are following diets as directed by physicians
  • Provides assistance in waking, bathing, feeding or toileting the residents as necessary
  • Assures residents are ready for appointments as scheduled
  • Monitors and provides assistance as needed in the dining area
  • Directs and supervises nursing assistants’ day to day functions
  • Ensures residents’ care is provided by the appropriate nurse or designee in accordance with federal, state and facility polices
  • Communicates clear expectations to nursing assistants on a daily basis
  • Provides leadership by example
  • Gives and receives report for the residents’ care each shift and communicates necessary information to nursing aides
  • Assists the DON reviewing and evaluating nursing assistant’s performance periodically and assists with staffing issues
  • Checks staffing board at the beginning of each shift and makes staffing readjustments as necessary
  • Reports staffing problems to the DON
  • Ensures that adequate stock levels of medications and treatment supplies are available to meet the needs of the residents
  • Recommends to the DON needed equipment and supplies on a daily basis
  • Maintains work areas in a clean and orderly fashion
  • Maintains all medication and treatment in a clean and orderly fashion
  • Keeps work area and utility rooms clean throughout the shift
  • Observes all infection control polices according to federal, state, and facility policies

Qualifications


  • RN degree from an accredited school of nursing or college/university: or
  • Graduate of an approved LPN program
