Our Supports Planners/Case Managers perform tasks to assess clients needs, situations, strengths, and support networks to determine their goals. As a Supports Planner, you will serve as the primary point of contact with clients. Additionally, you will receive remote training and access to the tools and resources needed to perform your role functions. Your responsibility will include information gathering, monitoring plans of service, and completing telephonic care coordination with participants.
