Big Bend Hospice is Hiring a Spiritual Care Counselor, Full-Time Near Tallahassee, FL

We are looking for a Full-time Spiritual Care Counselor to join our Big Bend Hospice (BBH) team! As a BBH Spiritual Care Counselor, you will provide spiritual counseling and emotional support to hospice patients and families based on the patient/family’s spirituality, religion, belief system, or philosophy of life, while remaining a neutral presence. Our Spiritual Care Counselors actively support, empower, and encourage connection in ways comfortable and familiar for the patient/family throughout their time on hospice services.


Requirements:

  • Master of Divinity Degree or equivalent from an accredited college. Acceptable concentrations would include, but not limited to theology, study of sacred texts, medical or professional ethics, psychology, sociology, family systems, counseling, social work, nursing, world religions, and gerontology.
  • Endorsement of a faith tradition, ordination, or seminary graduation is preferred.
  • Minimum of (1) Clinical Pastoral Education unit within a health care setting. (4 units preferred)
  • Preferred hospice, hospital, or clinical setting experience, but not required.


Travel Requirement
- You must have a reliable vehicle and be willing to be dispatched throughout the 8-County service area.

Our Spiritual Care Counselors go directly to the patient's homes, hospitals, facilities, or wherever the patient calls home to provide support and compassionate care.

Main Duties and Responsibilities:
(Includes, but not limited to)

  • Assess, identify, and create a care plan for the spiritual/emotional needs of patients and/or families.
  • Develops, initiates, implements, and evaluates a comprehensive spiritual care plan for the achievement of patient, family, and caregiver goals and for regulatory compliance.
  • Completes life reviews with patients and educates them on the dying process and end-of-life stages while reinforcing the plan of care.
  • Assists with Advance Directives upon request.
  • Offers and provides pre- and post-mortem sacramental ministry and rites of the sacred upon request of pt/family or arranges for appropriate clergy/support persons to do so.
  • Contacts outside clergy/support persons or faith communities on behalf of the patient or family as requested. Documents in EMR contact and a progress note. Completes follow-up as needed.
  • Maintains an effective spiritual care relationship with patient/family/caregiver. Supervisory visits reflect effective relationships based on rapport and methods of communication to provide appropriate spiritual care.
  • Plans and officiates at funerals, memorial services, and other services for the families/patients of 7 Oaks Healthcare/Big Bend Hospice as requested.
  • Communicates and collaborates with the interdisciplinary team about spiritual, ethnic, cultural, ethical, and grief issues of patients and families.
  • Clearly documents all patient/family contacts to include assessment, observations, interventions, evaluations, and outcomes on all spiritual care forms including, but not limited to Assessments, Progress/Clinical Notes, Care Plan, and Discharge/Death Summaries.
  • Completes a minimum of one face-to-face visit each month with a different faith community leader to inform/educate re: 7 Oaks Healthcare/Bid Bend Hospice.
