Big Bend Hospice is looking for a proven professional and hospice-minded Registered Nurse Manager to lead and develop a patient care team. In this position, you would lead and manage the Gadsden/Liberty Counties Homecare Team.
The RN Team Manager will direct all aspects (staffing, financial performance, and quality) of the care provided by the Interdisciplinary Group (IDG).
The RN Team Manager is the leader of the Patient Care Team. The primary function is to supervise and manage clinical resources for the Big Bend Hospice Interdisciplinary Group (IDG) and determine the daily utilization of staff and resources. Enhances clinical practice of the team by performing as a role model, mentor, instructor, and resource person.
Supervisory Responsibilities:
This position is responsible for supervising members of their assigned Team (i.e. RN, LPN, CNA, Social Workers, Music Therapist)

Primary clinical and administrative resource for the Interdisciplinary Group (IDG).
Determines the daily utilization of IDG staff.
Appropriately triage clinical calls to IDG members.
Provides coverage/assistance to team members as needed and as patient acuity dictates to ensure continuity of care to all patients on the team.
Participates in the team responsibility of providing 24-hour/day, 7-day/week patient care as needed.
Participates in clinical on-call staffing as needed.
Promotes adherence to Big Bend Hospice [BBH] policies and procedures. Recommends alternate approaches to patient care as indicated.
Obtains report and monitors ongoing assessments of physical, psychological, social and spiritual needs of the patient, family and patient caregiver and initiates appropriate intervention or referral in collaboration with IDG.
Provides education to IDG members regarding symptom control, case management, BBH, community, and financial resources.
Enhances the clinical practice of IDG staff by performing as a role model, mentor, instructor and resource person
Assists in determining the appropriate financial classification for patients throughout their continuum of care; i.e., hospitalizations, respite, nursing home placement, revocations, etc.
Responsible for daily operations of the Interdisciplinary Team (IDG).
Schedules on-call staffing.
Plans, organizes and coordinates IDG, including weekly IDG meetings.
Maintains current patient Kardex at all times.
Prepares the IDG agenda.
Collaborates with other departments, On-Call Nurse and Admissions, to ensure all patient/family care needs are met.
Communication
Communicates in a clear and accurate manner. Provides constructive feedback and resolves conflict directly. Assists others to improve group communication skills.
Assists in providing assessment, planning, implementation and evaluation of team goals.
Incorporates joint decision-making and unified goal achievement into IDG practice.
Demonstrates respect and understanding for all clinical disciplines required for quality patient care.
Responsible for Oversight of Staff Performance and Development.
Recognizes, reinforces, and enhances the role of all disciplines in the IDG. Promotes the professional development of all members of the IDG.
Assists in reviewing patient care documentation and providing on-site clinical supervisory visits to identify strengths and areas of development for the RN/LPN, Home Health Aides [HHA] and Homemakers [HMKR].
Assists team members in collecting data related to monitors identified.
Provides data and findings for reporting to CQI Committee.
Assists in the development of team-based education programs for the improvement of patient care.
Gathers written input to the evaluation of IDG from the respective discipline coordinator, i.e. FSC, Spiritual Care or Music Therapy.
Encourages staff to pursue certification and actively attend professional organization meetings.
Engages Actively in Professional and Leadership Development.
Identifies specific career goals that are consistent with team goals and BBH mission. Accountable for completion of goals at yearly evaluation.
Actively participates in a professional organization.
Demonstrates self-motivated professional and personal development by assuming responsibility for continued learning.
Administers direct care to select patients, as deemed appropriate, to maintain own clinical competence and/or to access patient requirements and to provide modeling for staff
