Job Description

Job Title: Registered Nurse Hospice Case Manager RN

Job Summary:

We are seeking a compassionate and experienced Registered Nurse Hospice Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and providing care to hospice patients, ensuring they receive the best possible support during their end-of-life journey.

Responsibilities and Duties:

• Develop and implement individualized care plans for each hospice patient
• Coordinate care services with interdisciplinary team members, including physicians, social workers, and chaplains
• Provide direct patient care, including symptom management and emotional support
• Conduct regular assessments and evaluations of patient progress
• Educate patients and families on end-of-life care options and resources
• Maintain accurate and up-to-date patient records
• Ensure compliance with all regulatory requirements and best practices in hospice care

Qualifications and Skills:

• Valid RN license
• Minimum of 2 years of experience in hospice or palliative care
• BSN degree preferred
• Certification in Hospice and Palliative Nursing (CHPN) is a plus
• Excellent communication and interpersonal skills
• Strong organizational and time management abilities
• Compassionate and empathetic approach to patient care
