Behavioral Health Company is Hiring a Behavioral Health Case Manager Near Mayer, AZ

Job Summary:

  • Calling and enrolling patients into Case Management program, and obtaining all medical and mental health history, completing any initial mental health assessment forms with patient during initial enrollment
  • Assessing patients needs and support systems
  • Contacting patients monthly for follow-up care
  • Assisting patients with scheduling medical appointments, contacting providers for medication refills, providing community resources, etc..
  • Assisting patients connecting them or assisting them with any community resources such as housing, obtaining ID, or things of that nature
  • Complete daily charting, and detailed documentation of all services provided and discussed with patients

KNOWLEDGE, SKILLS AND ABILITIES:

  • The knowledge of basic behavior modification principles
  • Provide treatment that promotes client dignity, independence, individuality, strengths, privacy and choice
  • Promotes a recovery-based approach to care that emphasizes hope, respect, self-direction and empowerment
  • Capacity for empathy, excellent time-management and communications skills
  • Creative problem-solving, and ability to multitask
  • Quick decision making, be organized and work well independently or as part of a healthcare management team.
  • Good time management, and communication skills

HIRING REQUIREMENTS:

  • Must be at least twenty-one (21) years of age
  • Highschool Diploma or GED
  • Valid drivers license
  • Level 1 fingerprint clearance card/First Aid & CPR certification
  • 1 year of experience in Behavioral Health (Preferred)
  • Knowledge of Windows, basic email, Microsoft word, timekeeping software, and basic computer navigational skills.
  • Ability to learn to navigate, and effectively complete documentation using our medical EMR system, eClinicalWorks
  • Self-motivated and self-directed

Job Details:

  • This job will be done on-site at our clinic in Chino Valley, AZ.
  • Shift Availability: Mon-Fri 7am-4pm with 1 hour lunch, 7:30am-4pm with 1/2 hour lunch.

Job Type: Full-time

Pay: $17.00 - $19.00 per hour
