General Summary:
Examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The Nurse Practitioner works within a collaborative model to provide quality care and to improve patient outcomes by strengthening continuity of care in all settings for patients, families, and staff in both the hospital and physician practice settings.
Responsibilities:
1. Accurately performs history and physicals for patients. Utilizes sound clinical judgment based on best practice when implementing the therapeutic regimen.
2. Documents all care provided, including pharmacological dispensing, in an accurate manner.
3. Completes patient discharge planning and education; writes appropriate orders and prescriptions. Performs patient education and counseling to promote wellness.
4. Evaluates patients progress toward attaining expected outcomes. Evaluates effectiveness of interventions in relation to outcomes. Documents patients response to interventions.
5. Maintains accurate, legible, and confidential medical records in accordance with federal and state laws.
6. All other duties as assigned, within the scope and range of job responsibilities.
