Delaware Hospice is Hiring a Nurse Practitioner (PRN) New Castle and Kent Counties Near Dover, DE

Delaware Hospice is actively seeking a PRN Nurse Practitioner. This Nurse Practitioner will serve hospice homecare functions, i.e., F2F visits, medical call, and provide community-based palliative care.

QUALIFICATIONS: Current license to practice professional nursing in the State of Delaware (in New Castle County, must maintain PA license as well). Graduate of an approved school of nursing. Bachelor's degree in nursing preferred. Certification in hospice nursing preferred. Minimum of two years of nursing, preferably in hospice or home care.
RESPONSIBILITIES: Provides palliative care and consultations as a partner in an inpatient care setting. Functions as a member of the palliative team to provide palliative services to patients with serious illness and their families. Assesses physical, psychological, emotional, spiritual and social needs of the patient and family. Makes recommendations for and prescribes medications and other therapeutic forms of treatment in an effort to mitigate symptoms related to serious illness. Documents visits and discussions using an EMR system. Communicates findings and recommendations to the care team, including consulting and attending physicians. Participates in the discharge planning process to assure safe transitions across the care continuum. Keeps current on issues involving goals of care conversations, Advance Health Care Directives and DMOST. Participates in staff and community education related to palliative care issues and end of life care. Relies on extensive experience and judgment to plan and accomplish goals. A wide degree of creativity and latitude is expected. This is an exempt position. Must be able to bend, squat, lift and turn patients, able to lift 30 to 60 lbs, following industry standards and guidelines for safety and correct ergonomic practices. Employees involved in patient care may be exposed to blood, body tissue and fluids regularly and are expected to follow all health care industry safety guidelines indicated in our procedures manual.
