Barnes Health Care Management Group LLC is Hiring a Care Manager Near Fallon, MO

Description

Are you an enthusiastic, patient centered nurse that resides in the state of Missouri? Join our team of Care Managers that specialize in providing care for aging adults. Our team provides chronic care management services over the phone for patients that are located throughout the state of Missouri You'll be managing the care for seniors living in skilled nursing, assisted living and other long term care facilities. You MUST be a licensed nurse to be considered for this position. Must be able to travel to Columbia, St. Louis or Kansas City.

Highlights:

  • Day shift hours
  • Casual attire every day
  • No more heavy lifting or call lights!

Preferences:

  • Active LPN license in good standing
  • Preferred experience with skilled living communities
  • Preferred experience working with computers/software
