Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will provide direct patient care under the supervision of a registered nurse or physician. Your primary responsibility will be to ensure the well-being and comfort of patients while adhering to healthcare protocols and standards.
Duties:
- Administer medications, treatments, and vaccinations as prescribed by healthcare providers
- Perform wound care, including cleaning, dressing, and suture removal
- Monitor patients' vital signs and report any significant changes to the healthcare team
- Assist with physical examinations and provide support during procedures
- Collaborate with registered nurses and physicians to develop and implement patient care plans
- Educate patients and their families on health conditions, medications, and self-care techniques
- Provide emotional support to patients and their families during challenging times
- Assist with discharge planning and coordination of follow-up care
- Document patient information accurately and maintain confidentiality of medical records
- Utilize electronic health record systems such as Athenahealth, Epic, or other relevant platforms for documentation and coding purposes
Experience:
- Must have a valid LPN license in [state]
- Previous experience in a healthcare setting preferred
- Familiarity with working with people with developmental disabilities is a plus
- Knowledge of family planning principles and practices is beneficial
- Proficiency in discharge planning procedures is desirable
- Experience with Durable Medical Equipment (DME) is an asset
