Description

Job Description:

Maintains a calm, recovery-focused, safe, and sanitary environment of care within the residential milieu. As a direct care professional, encourages individuals receiving care to participate in both structured and unstructured therapeutic activities. Actively monitors and engages individuals receiving care, documenting and tracking activities of daily living, interpersonal functioning, and progress toward individual treatment goals. Participates in reviews progress notes, assessments, and evaluations.

Essential Functions:

  1. Supports the delivery of care and engagement of individuals under care by participating in group supervision and by reviewing evaluations, assessments, treatment plans, and progress notes. to understand the unique needs, strengths, preferences, and goals.
  2. Monitors and documents the health and safety of individuals in care at scheduled intervals, including common rooms, restrooms, bedrooms, cafeterias, therapy/case management offices, and outdoor areas.
  3. Models pro-social interactions and encourages participation in therapeutic activities in individual and group settings.
  4. Following universal precautions and health protocols, maintains a clean and healthy environment of care through sanitation activities, including but not limited to: trash removal, wiping frequently touched surfaces and work areas, laundry, cleaning of dishes/utensils, and occasional spills or messes.
  5. As approved and assigned, uses company vehicles to provide transportation for individuals entering, receiving, or discharged from care.
  6. Warms and serves meals and food items following health and safety protocols, following documented medical orders for specialty diets and restrictions by the medical practitioner.
  7. Monitors and assists with activities of daily living.
  8. Serves as a liaison as assigned and appropriate for internal and external stakeholders, care partners, and visitors.
  9. Demonstrates adherence to company policies, expectations, and behavioral competencies, both explicit and implicit.
  10. All tasks, projects, and/or duties as assigned and authorized, as determined by Human Resources and Direct Supervisor (or designated delegate) to be congruent with the role.

Qualifications


Education:

High School Diploma, GED, or Equivalent, Required

Associate Degree in Human Services or related field, Preferred

Experience:

One year of work or engagement experience working with individuals living with dynamic and complex behavioral health needs, Required

Additional:

Valid and unrestricted driver’s license, Required

Proficiency with Microsoft Office Suite Products, Required

Proficiency and familiarity with EHR/EMR systems, Preferred

Credentials:

Certified Health Worker, Preferred

Mental Health First Aid Certification, Preferred

Physical Demands:

Prolonged periods of sitting at a desk or computer workstation.

Prolonged periods of standing, bending, and reaching.

Must be able to lift weights up to 50 pounds at times.

Psycho-Social Demands:

Seeks guidance / help proactively to support safety and quality care.

Demonstrates skill of distress-tolerance to maintain calm and safety.

Models positive regard with all members of the community of care.

Establishes and maintains interpersonal boundaries.

Maintains personal and situational awareness through self-reflection.

Practices ethical, evidence-based, and person-first decision-making.
