RN for Assisted Living

Under the general supervision of the Administrator/Supervisor of the respective agency, the Licensed Practical Nurse who provides intermittent, scheduled, skilled nursing care to clients in their home promoting and maintaining their health. Their role entails: prevention, restoration, rehabilitation and education. The Primary Care Nurse will service the Assisted Living campus.

Responsibilities:

Provides skilled nursing services in the resident’s home on campus including the following which shall be documented in the client’s clinical record:

• Evaluation of clients' needs and development of a client care plan.
• RN admits and recertifies clients.
• Develops a plan of care and instructions for the ancillary personnel involved in their client’s regime.
• Coordinates the care of services for clients with other health care professionals including: other primary care nurses, home health aides/patient care assistants, therapists or companions.
• Instructs, supervises and evaluates the plan of care under the direction of the client’s physician.
• Interprets for the client and family their diagnosis, implications and treatment consistent with the physician.
• Reviews and supervises plan of care, involving home health aides, patient care assistants and companions.
• Visits and evaluates the clients for progression toward goals and termination of services if and when appropriate.
• Facilitates access of community resources as appropriate.
• Collaborates with other agencies to manage client needs as appropriate.

EXPERIENCE:

• Current RN license in good standing with the State of CT
• Home health care or Assisted Living experience preferred.
