MDS/Resident Assessment Coordinator (RN or LPN)

Primary Function/General Purpose of Position

Conduct and coordinate and complete MDS and care plans for residents in accordance with the requirements of this state and the policies and goals of this facility. As a Resident Assessment Coordinator, you ‘ll be working exclusively in the Resident Assessment Coordinator Department, no on call etc.

Employment Qualifications

Education: Must possess, as a minimum, a Nursing Degree from an accredited college or university.

Licensure: State of Florida LPN or RN nursing license required.

Experience: Must have a minimum of 1 years’ experience with the PDPM and RAI process.

Essential Job Functions

• Works with the interdisciplinary team in filling out the MDS and preparing the care plans for all residents accurately and on a timely basis.
• Acts as a Resource to other staff regarding the MDS/PDPM/CP process.
• Schedules the MDS and care plans for residents on your assignment
• Ensures resident and family involvement in the development of the plan of care if able.
• Reviews the MDS/CAA’s/CPs for accuracy and bring concerns to the interdisciplinary team members or Director of Nursing.
• Ensures that care plans are kept up to date.
• Works with Facility Coder to insure accurate diagnosis are recorded in resident’s record.
• Act as backup for Department Manager at meetings.
• Ensure that resident interviews are done timely and conducted in private.
• Work as a team member with other Resident Assessment Coordinators
