Immediate opening for an experienced Nurse Practitioner working in Primary care in Richmond.

Why this job:

• Days, M-F schedule but there may be very occasional consultative calls on the weekends, but expect that to be light to non-existent.
• You will see patients with Serious Mental Illness being responsible for supporting, caring for and documenting their medical primary care needs .
• Perform whole-person clinical assessment to identify and remediate care gaps. On an ongoing basis, you will support care coordination among the individual’s medical and behavioral health providers, making referrals as appropriate, as well as provide clinical support to Guides (peers) and Community Resources Guides (social workers).

What you'll need to qualify: Must have at least 3 years clinical care as a Nurse Practitioner, s eeking candidates with solid Primary Care skills and experience. You need to be comfortable working with and managing underserved populations with challenges such as behavioral health and/or SUD conditions. Developing and delivering educational programs to nurses and affiliated medical staff. Care management and coordination. Current Virginia license
