Position Summary:As a Hospice Team Leader, you will lead a dedicated team in providing interdisciplinary services to patients and families, contributing to their comfort and well-being. Your role will involve overseeing daily operations, ensuring compliance with clinical standards, and fostering a culture of empathy, respect, and teamwork. Duties and Responsibilities:
Ensure personalized care adhering to established standards, enhancing patient comfort and dignity
Recruit, mentor, and guide staff, providing comprehensive orientation and ongoing education
Maintain compliance with policies, regulations, and fiscal responsibilities
Cultivate an environment of compassion, respect, and trust among team members
Collaborate closely with other leaders to ensure seamless coordination of care
Required Skills/Abilities:
Exceptional critical thinking and nursing judgment
Strong problem-solving abilities with timely, effective solutions
Organizational prowess and attention to detail
Ability to work autonomously and exhibit self-direction
Outstanding interpersonal and communication skills
Comfort in facilitating crucial conversations with patients, families, and staff
Empathy and active listening skills to meet diverse needs
Ability to ensure compliance with regulations
Qualifications:
Graduate of an accredited school of nursing (BSN preferred)
Unrestricted Registered Nurse license in Texas
Operational and personnel management experience (preferred)
Valid Texas driver’s license, auto liability insurance, and safe driving record
Current CPR certification
Knowledge of hospice regulations and previous experience preferred
Proficiency in Microsoft Office and electronic medical records (EMR)
