Description

Aurora Mental Health & Recovery (AMHR) is seeking a Patient Care Coordinator to provide comprehensive patient care coordination to severely persistent mentally ill adults and to adults with substance use disorders. The care consists of medical case management and care coordination. The ideal candidate is a self-starter, embodies integrity and humility, and has a personal sense of responsibility and passion for underserved populations. Shift. We are hiring full-time Patient Care Coordinators to work Monday–Friday, 8:00am-5:00pm. We have a hybrid work schedule where you can enjoy working remotely up to 3 days per week!

About the team.

The Patient Care Coordinator will be a part of our interdisciplinary Physician Operations team and supports all clients receiving medication services at Aurora Mental Health and Recovery. This includes children, adolescents and adults, as well as clients with substance use disorders.

$2,000 Sign On Bonus Available! *Not eligible for PRN candidates, internal employees, and re-hired employees within 90 days from separation.

Based on full time (40 hours/week). Prorated if fewer than 40 hours/week. Paid in two installments: 50% on first paycheck, 50% on paycheck following 6-month anniversary.

Salary for this role. Starting wage is based on experience and company equity. Paid bi-weekly.

  • $19.65 - $29.46 per hour 

Bilingual Language Differential Pay. *Rate depends on language requirements for the position and candidate's fluency.

  • Will receive scheduled earnings on a bi-weekly basis for all hours worked (excluding non-productive hours paid but not worked, e.g., paid time off, holiday, administrative, etc.) in an amount equivalent to 5% of their regular base rate (hourly or salaried), to a maximum amount of $3,000.00 per calendar year. *The annual maximum amount will be prorated based on full time equivalency (FTE).

Responsibilities:

  • Foster a culture of patient-focused care, understanding that the quality of the relationship is a key factor in healing.
  • Actively collaborate and communicate with staff across the agency to provide outstanding patient care.
  • If authorized and appropriate, cultivate positive relationships with each patient's outside providers to enhance care continuity and integration.
  • Promote, encourage, and model the practice of continuous improvement with a focus on patient experience, access, safety, and treatment effectiveness.
  • Help clients evaluate strengths and symptoms. Facilitate in clients setting their own goals and plan for appropriate services.
  • Offer support, assistance, consultation and education to families, friends, landlords, employers, community agencies and others who come into contact with clients (case management) in order to maximize benefits and minimize problems associated with the presence of these persons in the community.
  • Reach out to eligible clients to inform them of and educate them about available services at the organization and in the community.
  • Understand and comply with organizational goals around quality, compliance, standards, and policies.
  • Complete required education and training in a timely manner.
  • Accurately and completely document patient care (CCARS, COMPASS, etc.)
  • Provide information to Credentialing, Payroll, etc. in a timely manner.

Qualifications


Requirements:

  • Bachelor's degree.
  • Preferred: Master's degree.**
  • Preferred: three years of relevant clinical work experience.**
  • Advanced computer skills, including use of electronic health record, MS Office products and the ability to learn new tools as needed.
  • Ability to exercise independent judgment and discretion.
  • Excellent written and oral communication skills.
  • Strong customer service and interpersonal skills.
  • Ability to organize and prioritize workload.
  • Detail-oriented with ability to multitask.

Required Vaccinations and TB Test.

AMHR has prioritized the health and safety of our clients and staff and requires all employees to be fully vaccinated against Covid-19, receive an annual influenza (flu) shot, and receive negative TB test results prior to their first day of hire. Proof of both vaccinations and negative TB test will be required upon your first day of employment. Medical and religious exemptions may be requested for the COVID-19 vaccine, annual flu shot, or TB test.
