Hours to align with patient needs during clinical hours of operation: 8am to 5 pm in the form of two full days and one half day per week, or 2 days/3 days alternating weekly.12-month assignment, with annual renewal based on program performanceJob OverviewThe Clinical Nurse Care Coordinator is responsible for facilitating and managing care coordination of designated populations of the most complex pediatric patients within an ambulatory environment. Leads internal and external care team partners in the delivery of high-quality, safe, personalized, and cost-effective, outcomes-based, family-centered care.The ImPACT Hub is a new initiative at CHCO that will provide enhanced support for adolescents and young adults (AYA) and their families transitioning from pediatric to adult care. The ImPACT Nurse Care Coordinator (NCC) will work closely with a Family Navigator and Social Worker to develop and deliver high quality, patient/family centered evidence based adult transition care. Facilitates and documents changes and/or interventions as needed.Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health.Creates ongoing processes for patients/families to determine and request the level of care coordination support they desire over time.Facilitates patient access to appropriate medical and specialty providers as well as other care coordination support, including, but not limited to, medication acquisition and management, procedures/tests, appointments, and referrals.Serve as a point of contact, advocate, navigator, and informational resource for patient, family, care team, payers, and community resources. Facilitates related meetings as needed.Provides mentoring/coaching to other population health and care coordination team members.Participates in practice transformation efforts that support ongoing coordination of care for medically complex patients and families through sharing knowledge, acting as role models, and providing technical assistance.Participates in the development and revision of policies and procedures, as well as with quality improvement and research activities impacting care for designated patient populations.Utilizes information available within the organization in conjunction with technology to support communication, clinical decision making, error prevention, and overall care coordination.Other InformationSCOPE & LEVELGuidelines: Full performance professional level work that requires additional knowledge, education, training or experience.Complexity: Duties performed involve concepts, theories, and concrete factors to be evaluated and weighed requiring a high degree of analytical ability, independent judgment, and decision-making. Work assignment is generally unstructured and employee is responsible for organizing complex, varied, and simultaneous coordination of several functions, programs, or projects in various stages of completion.QualificationsEducation: Bachelor's degree in nursing (BSN) is required.Experience: Minimum of four (4) years of nursing, case management or care coordination experience in a pediatric environment. Experience with pediatric and/or adult bleeding disorder patients preferred. Certification(s):BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire.Achievement and maintenance of Care Coordination and Transition Management (CCTM) certification within six (6) months of hire or transition into role. Successful completion of internal Care Coordination training within six (6) months of hire or transition into role. Licensure(s): Registered Nurse (RN) licensure in the state of Colorado is required. Professional Membership(s): Active membership in a professional nursing or healthcare organization is required.Physical RequirementsAbility to Perform Essential Functions of the JobBody Movement: Will frequently stand/walk up to 12 hours per day.Body Movement: Will frequently use hands while keyboarding and writing.Body Movement: Will frequently sit up to 2-4 hours per day while accessing computer, talking on the telephone, and while attending meetings.Back and Upper Body (lifting and carrying): Will occasionally lift/force exerted up to 100 pounds.Vision: Constantly uses sight in performance for daily job duties.Hearing: Constantly uses hearing in performance for daily job duties.Others: Able to assist or assume direct patient care as census directly permits.
