Leads internal and external care team partners in the delivery of high-quality, safe, personalized, and cost-effective, outcomes-based, family-centered care.The ImPACT Hub is a new initiative at CHCO that will provide enhanced support for adolescents and young adults (AYA) and their families transitioning from pediatric to adult care. The ImPACT Nurse Care Coordinator (NCC) will work closely with a Family Navigator and Social Worker to develop and deliver high quality, patient/family centered evidence based adult transition care.
