Job Description:

The Care Coordinator Lead plays a pivotal role in the organization's efforts to ensure patient navigation is executed by managing client caseloads. The primary focus of this role is to ensure patients are engaged with their Primary Care Provider (PCP) at least annually and have a visit for any transition of care or hospital follow-up needs. This role also educates patients, facilities conversations between care teams, aggregates data, supports a team, and identifies areas for improvement.

Responsibilities:

• Establish trust and rapport with new patients, existing patients, families, and caregivers
• Advocate for patient rights and ensure that they receive the best possible care, including coordination of care, referrals, and community resources. Such as appointments for specialists, Non-Emergent Medical Transportation, and Medicaid & Medicare applications
• Conduct needs assessment, treatment planning for patient-centered care plans, and follow up to ensure care and education needs are met
• Navigate through appropriate interventions such as de-escalation, Crisis Prevention, motivational interviewing, etc.
• Plan, track, and audit outreach to meet Transition of Care visit goals
• Lead and foster relationships with internal and external stakeholders, facilities, and payors
• Partner in Admission, Discharge, Transfer feeds
• Prepare regular KPI and KAI reports and presentation on work activities for management
• Incorporate Provider and patient feedback into improvement strategies for development of education materials and tools Facilitate the development and updates of Standardized Operating Procedures and guidelines
• Project management improvement plans and strategies
• Collaborate with Care Coordinator and other healthcare providers to ensure patient safety and quality care
• Complete, document, and report daily activities, telephone call, department, and patient meetings, etc.
• Ability to manage a daily caseload of a minimum of 15 patients
• Provide day-to day support, supervision, and performance reviews for team
• Complete additional tasks and duties as assigned.

Requirements:

• Bachelor’s degree in community health worker, social work, healthcare administration, or a related field
• 2 years of experience in care/case management
• Excellent interpersonal and leadership skills
• Ability to manage competing priorities and needs of multiple stakeholders
• Knowledge of healthcare regulations and best practices
• Ability to motivate others, individual, and groups
• Strong organizational, critical thinking, and problem-solving skills
• Ability to analyze, develop and present high level analysis and metrics
• Ability to foster a healthy work culture and ability to work alone or in a team
• Capability to work in a fast-paced environment and adapt to new policies or workflows
• Ability to maintain patient confidentiality
• Proficiency in Microsoft Office (Excel, Word, Outlook) and electronic medical records
• Community Health Worker license preferred
• Medical Assistant certification preferred
• RN/LPN license preferred
• Bilingual in Spanish preferred
• 2 years of experience in Primary Care/Medical Group preferred

Up to 50% travel may be required.

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
