Grant a Gift Autism Foundation is Hiring a Care Coordinator/Case Manager Near Las Vegas, NV

The Care Coordinator will provide quality case management for families as they navigate the systems necessary to support their child with autism or other neurodevelopmental conditions. The Care Coordinator is responsible for guiding families through programs and services, consulting with families and determining their needs, coordinating with the clinical teams on care plans, educating them about autism and other neurodevelopmental conditions, and working with the clinical team to evaluate interventions. This role requires strong organizational, communication, and interpersonal skills to coordinate with patients, healthcare providers, and other stakeholders effectively.

Duties and Responsibilities

  • Deliver high-quality, patient-centered case management support to all patients and their families.
  • Develop a rapport with families by facilitating thorough communication from initial contact to post-service follow-up.
  • Accurately document patient needs and care coordination activities within an Electronic Medical Record, to ensure team members can track on-going care.
  • Comply with all relevant privacy, confidentiality regulations and organizational guidelines.
  • Attend ongoing training and courses to keep up with new developments in healthcare, within the disability field.
  • Identify and follow-up on all referrals made to assure continuity of care and patient/family needs are met.
  • Maintain open and clear communication with patients, families, and healthcare providers.
  • Assist with referral process from and to community agencies through the development of relationships and partnerships.
  • Research, attend and participate in local resource fairs.
  • Assist with the coordination of program orientations, parent training, sibling and young adult groups, and other family related workshops and training.
  • Support clinical team during FASD/Drug Effect Clinic and Psychoeducational Groups.
  • Assist Spanish speaking patients in navigating the healthcare system, addressing barriers they may encounter.

Educational Requirements

  • Bachelor's degree or licensure in social work, psychology, or similar field

Qualifications

  • Compassionate and patient-centered approach to care coordination.
  • At least 1 year of experience working with low income and/or vulnerable populations – does not have to be autism/neurodevelopmental conditions related.
  • Ability to discuss sensitive issues with families using compassion and empathy.
  • Experience and working knowledge of healthcare regulations, and federal support systems (e.g., Medicaid, Medicare, SSI, and other government and commercial insurances).
  • Experience working within a non-profit, clinical training center (Preferred)
  • Strong working knowledge of Microsoft Office, especially Excel, Word and Outlook.
  • Excellent written and oral communications skills.
  • Creative, professional, and resourceful with the ability to work independently and a part of a multidisciplinary team.
  • Ability to take initiative and complete multiple tasks and projects on time.
  • Previous experience working within an Electronic Medical Record (EMR).
  • Detail orientated and strong organizational skills.
  • Bilingual in English and Spanish a plus.

Physical Requirement

  • May include standing, sitting, and/or walking for extended periods

  • May include lift up to 50 pounds
  • May require ability to transport from one site to another
