Positive Leaps is hiring full-time Community Case Manager to join our West Chester team!

About Us:

Positive Leaps LLC specializes in helping children aged 3-8 who demonstrate negative behaviors or need help with emotional regulation. Our Day Treatment Center mimics the look and feel of daycare or early learning center. We provide high staff-to-child ratio, helping children learn emotional and social skills to get them back on track. No former experience working with children is required - we provide all the training necessary for the role!

Position Overview: Provides a safe and effective therapeutic environment for all clients in accordance with the philosophy and Mission of Positive Leaps. Responsible for leading care coordination in collaboration with internal and external supports to ensure cohesive and effective treatment as well as confirm supports are available and active for clients and families to reduce barriers to treatment in an effort to achieve excellent clinical outcomes.

Essential Duties:

  • Provides case management services to families of all ages of children in community settings, such as schools, daycares, etc. as well as in the home, in the office, and virtually as needed.
  • Assist families in symptom monitoring, case coordination, resource identification, and with other social and treatment needs.
  • Facilitates Prior Authorization and Re-Authorization contact with 3rd party payers, as needed, to ensure financial coverage for clients in treatment.
  • Serves as a point of contact for families, treatment team members, collaborative agencies, and community support.
  • Regularly meets with supervisors and/or treatment team to plan and problem solve for client and family needs.
  • Provides a safe space for clients and families.
  • Interacts with children, families, and other staff members in a professional, positive manner.
  • Exhibits flexibility and adaptability in rapidly changing environments.
  • Supports prescribed clinical treatment by teaching and implementing the Positive Leaps model of care and curriculum or any other prescribed intervention.
  • Prepares, updates, and maintains timely and accurate documentation of clinical data in accordance with Ohio Laws, 3rd party payers, and agency policies.
  • Implements and collects data related to treatment goals and objectives.
  • Consistently meets performance measures including competency indicators and participates in quality assurance activities (data gather, data analysis) for the purpose of continuous quality improvement.
  • Assures consistent implementation of Positive Leaps’ Policies and Procedures.
  • Provides crisis intervention services during or after regular business hours, if the need arises
  • Demonstrates ethical conduct, in keeping with professional standards set by licensee’s professional board and Positive Leap’ ethical code.
  • Models professional behavior and demeanor, dress and work space organization.
  • Completes assigned tasks as directed by supervisors and within agency timeframes.
  • Performs other duties as assigned.

Minimum Requirements: 

  • Requires a bachelor’s degree in Social Work with License
  • Must be at least 19 years of age.
  • Must be able to work with all ages of children. 
  • Must have excellent verbal and written communication skills. 
  • Ability to work in a very active environment and lift up to 50 lbs. 
  • Able to pass a rigorous background check.
  • Must possess a valid Ohio driver’s license and auto insurance.
  • Willingness to work in various environments to promote treatment (i.e. client home, school, office, and community)

Exposures and Risks:

  • Potential for exposure to blood and body fluids
  • Works directly with physically aggressive children
  • Exposed to loud noise, such as repeated and prolonged screaming and crying
