The Community Liaison will require interactions with physicians, hospitals, and community partners as we seek to provide continued delivery of care throughout the service area.
Essential job functions & responsibilities
•Your responsibilities would include communicating the benefits of hospice care to these professionals and families, educating the community about hospice services, and conducting sales calls to existing and potential referral sources.
Establish and maintain positive working relationships with current and potential referral and payer sources

Qualifications:
§ Works well within a team environment to collaborate daily with interdisciplinary team.
§ Can work independently and on your own on a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
§ Time management skills to manage visits, documentation, and meetings.
§ Ability to learn electronic medical records system.
§ Must have reliable transportation.
§ Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.
§ Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.
§ Strives for independence and sound critical decision making
