Ascend Community Staffing Partners LLC is Hiring a Crisis Case Manager Near Dalton, GA

Description

Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. Being part of our team and working in a CSB means you’re helping individuals and families otherwise unable to access treatment from private providers to address healthcare challenges associated with mental illness, intellectual/developmental disabilities, and addictive diseases. You’ll be working with purpose to make a difference in the lives of people who need you most. Each of our jobs will work within a State Community Service Board facility.

  • Completes admission assessments as assigned and documents them according to policy and procedure.
  • Ensures Individual Treatment Plans are completed timely as outlined in policy and procedure.
  • They must have individualized goals, that are reasonable and measurable.
  • Focus on 30 day re-admits regarding documentation on reason for re-admit and developing goals to prevent relapse.
  • Assist individuals with long term placement.
  • Provides daily groups and documents participation and response timely and as required by policy and procedure.
  • Progress notes regarding discharge planning must include progress or lack of progress towards discharge.
  • Participates in daily Treatment Team Meeting.
  • Essential Learning Courses as scheduled.
  • Enters connects reauthorizations as needed into computer system as required.
  • Meets with individual’s family members as needed for discharge planning and documents results of the meeting.
  • Completes all discharge planning documentation timely and as outlined in policy and procedure.
  • Ensures discharge appointments are made, including any medical follow-up needed and documents in the individual’s record.
  • Assist individuals with ensuring WRAP plans & SNAPS are completed.
  • Schedules transport when consumers are unable to obtain.
  • Other duties as assigned by supervisors.

Requirements

Bachelor Degree in a helping Field

1 – 2 years’ experience in mental health/ substance use disorder preferred

Ability to deal effectively with conflict situations, manages aggressive behavior, and uses non-violent crisis intervention and behavioral management techniques.

Must be able to pass an intensive criminal background investigation

Pre-Employment and random drug screening

Have an acceptable motor vehicle report
