Job Summary
This position conducts in-person assessments for individuals in need of community-based long-term services and support.
Essential Job Functions
Job Requirements
Licensed Social Worker or Licensed Independent Social Worker with at least one (1) year prior experience in home care for the elderly, home health care, medical social work, or gerontology. Knowledge of community resources. Reliable transportation, insurance, valid driver's license, background check, and drug/alcohol screening are required.
