DDD I/DD Agency Case Manager

Full Time, Mercer County NJ

Who We Need:

The Agency Support Specialist is designed to support activities within The Family Supports Department. The Agency Support Specialist oversees and coordinates the Interdisciplinary Team process and is the liaison between Support Coordinators, the Division of Developmental Disabilities, Families, and agency staff. This role will also facilitate agency growth through processing new program admissions.

What We Need:

• Independent, motivated professional with effective written and verbal communication skills.
• Strong, understanding of Individual Service Plans.
• Understand the process of irecord, ISP's and SDR's.
• Ability to work on multiple projects.
• Strong interpersonal skills and adept at speaking with families, DDD, and outside agencies.
• Strong organizational skills, multi-tasking, and problem-solving.
• Familiar with Electronic Health Records.

What You Need:

• Associate degree or higher.
• Three (3) years' experience in the field of Human Services.
• Three (3) years experience in Customer Service and /or Sales.
• Three (3) years of experience using Microsoft Office, Google Sheets/ Docs, and database administration in a business environment.
• Expert in computer and internet research skills.
