Job Summary:
We are seeking a dedicated Social Worker to join our team. The ideal candidate will have experience in various settings such as hospitals, counseling, and hospice care. This role involves patient assessment, working with medical records, and providing support to individuals with diverse needs.

We have needs in the Greater New Orleans area as well as the Northshore, Tangipahoa, the River Parishes, Washington parish and Baton Rouge.

Responsibilities:
- Conduct patient assessments to determine needs and develop care plans
- Maintain compliance with HIPAA regulations and ensure confidentiality of patient information
Work with adults and their families in hospice care

Qualifications:
- Bachelor’s or Master’s degree in Social Work or related field
- Previous experience in hospital or healthcare settings preferred
- Proficiency in medical records management and documentation
- Experience in addiction counseling and working with diverse populations
- Strong interpersonal skills and the ability to collaborate with multidisciplinary teams

Job Types: Part-time, Per diem, PRN

Pay: $64.00 - $65.00 per hour
