Job Summary:
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide end-of-life care and support to patients and their families in various settings.
Responsibilities:
- Manage patient care plans and provide direct nursing care in hospice settings
- Coordinate with interdisciplinary teams to ensure comprehensive care for patients
- Supervise and train nursing staff on hospice care protocols
- Conduct home visits to assess patient needs and provide education to families
- Collaborate with physicians to develop and implement treatment plans
- Review and update patient documentation regularly
- Provide emotional support and guidance to patients and families during difficult times
Requirements:
- Active RN license in the state of practice
- Prior experience in hospice care or a related field preferred
- Strong knowledge of physiology, palliative care, and pain management
- Excellent communication and leadership skills
- Ability to manage shifts effectively and prioritize tasks
- Familiarity with electronic medical record systems
- Compassionate attitude towards end-of-life care
