Clinical Associate I, II, or III - Assertive Community Treatment (ACT)

About the Team
The Assertive Community Treatment (ACT) team is a community based, multidisciplinary team, that supports some of the community's most acute individuals. The team primarily serves clients who experience schizophrenia spectrum disorders, often with co-occurring substance use disorders, and significant functional or life impairments. The work is fast-paced, unpredictable, and requires risk management within the ACT model. The ACT Team provides services to housed and homeless individuals with the goal of helping them to achieve and maintain their highest quality of life through a range of services to include but not limited to medication management, crisis intervention, skills training, and case management.
What You'll Do
* Provide case management support to clients in connecting to community resources as well as active skill building, focusing on skills to address functional impairments and to enhance the quality of life
* Assist clients in meeting overall goals as identified in treatment plan; to include areas of management of daily routine/skills building, basic home upkeep, employment, housing, education, and treatment program attendance

* Pay Range is $19.23 to $29.47/hour DOE (see below)
* This position is eligible for annual incentive opportunities

What We Need from You
Clinical Associate I - $19.23 - $24.89
* You have a high school diploma or GED
* You have one year's experience working in health care, home health, or mental health service settings with an adult population who has experience a severe and persistent mental illness
* Valid drivers license with a 5-year clean driving record
Clinical Associate II - $21.00 - $27.18
* High school diploma or GED. (Associate Degree applicants greatly encouraged to apply)
* Two years of case management experience working with mentally ill adults in relevant settings such alcohol, drug, developmental disability, physical rehabilitation, etc.
* Valid drivers license with a 5-year clean driving record
Clinical Associate III - $22.78 - $29.47
* Bachelor's degree in a human-services field such as social work, psychology, etc.
* Five years of relevant experience may be substituted for the degree.
* Valid drivers license with a 5-year clean driving record
Preference will be given to candidates who have completed Peer Support Training and have experience in a Clinical Associate/Case Management/Peer Support Role.
