Job Title: RN Director of Case Management for Acute Care
Job Overview:
We are seeking a highly skilled and experienced Registered Nurse (RN) to join our healthcare team as the Director of Case Management for Acute Care. In this leadership role, the successful candidate will be responsible for overseeing and optimizing the case management processes within our acute care facility. The Director will collaborate with multidisciplinary teams to ensure efficient and effective patient care coordination, resource utilization, and discharge planning.
Responsibilities:
Strategic Leadership:
Develop and implement strategic plans for case management services to align with organizational goals and regulatory requirements.
Provide visionary leadership to the case management team, fostering a culture of excellence, collaboration, and continuous improvement.
Operational Oversight:
Oversee the day-to-day operations of the case management department, ensuring the delivery of high-quality, patient-centered care.
Monitor and evaluate case management processes, identifying opportunities for enhancement and implementing best practices.
Care Coordination:
Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure seamless care coordination throughout the patient's stay.
Develop and implement care plans that address the physical, psychosocial, and financial needs of patients.
Utilization Review:
Implement and oversee utilization review processes to optimize resource utilization and ensure appropriate levels of care.
Collaborate with the finance department to manage length of stay, reduce unnecessary costs, and improve resource allocation.
Performance Improvement:
Establish and monitor key performance indicators (KPIs) related to case management outcomes.
Lead performance improvement initiatives to enhance patient outcomes, reduce readmissions, and improve overall efficiency.
Staff Development:
Recruit, train, and mentor case management staff, fostering a culture of professional growth and development.
Ensure staff compliance with regulatory requirements and organizational policies.
Interdisciplinary Collaboration:
Foster strong relationships with healthcare providers, payers, and community resources to support effective care transitions.
Collaborate with leadership teams to develop and implement integrated care models.
Qualifications:
Current Registered Nurse (RN) license in the state of practice.
Bachelor's degree in nursing; Master's degree in nursing or related field preferred.
Certification in Case Management (CCM) highly desirable.
Minimum of 5 years of progressive case management experience in an acute care setting.
Strong knowledge of healthcare regulations, reimbursement systems, and care coordination processes.
Excellent leadership, communication, and interpersonal skills.
Demonstrated ability to lead and motivate a multidisciplinary team.
