Amelia Home Care, Inc is seeking a dedicated Care Coordinator to join our team. The Care Coordinator will play a crucial role in supporting the coordination of care for patients. This position offers the opportunity to work in a dynamic healthcare environment.

Responsibilities:

- Staff open cases

- Coordinate patient care activities to ensure appropriate follow-up and adherence to care plans

- Communicate with the HHA/PCA'S on daily basis and follow up with them for missing timesheets

- Communicate with healthcare providers, patients, and families to facilitate continuity of care

- Document all notes on HHA exchange

- Responsible fro completing incident/update reports

- Dashboard mainteance: Monitor HHA exchange to confirm daily missed visits and weekly pre-billing updates

- Responsible for on call coverage to ensure HHA placement

- Assist in obtaining authorizations for medical procedures and services

- Provide medical administrative support as needed

- Collaborate with medical and administrative staff to optimize patient care

Qualifications:

- Must have home care coordinating experience

- Must speak fluent spanish

- Must have hha exchange knowledge

- Excellent communication , attention to details

- Ability to work effectively in a team environment
