Alvarado Care Center is Hiring a Program Counselor Near El Monte, CA

Program Counselor 

The essential functions of your job are the following:

·To build a therapeutic relationship with residents.

·Meeting with residents on caseloads for 1:1 at least monthly

·Initiate residents’ STP care plan centering interventions on the specific needs of the individual.

·To deescalate residents in crisis and provide support following the crisis.

·To provide support in clinic with psychiatrist.

·Provide nonpharmacological interventions to psychotropic medication care plan.

·Collaborate with Interdisciplinary Team for residents' well-being.

·Provide ample objective documentation on 1:1 meeting, residents behaviors, progress, and any other information of note.

·Help clients identify goals and potential solutions to problems pertaining to mental and behavioral well-being

·Seek to improve communication and coping skills, strengthen self-esteem and promote behavior change and optimal mental health for all clienteles.

·Responsible for planning, implementing and facilitating outcome-based activities required within program.

·Provide assistance to help clients develop daily living skills and improve their mental, physical, or emotional health.

·Sufficient verbal and written communication skills required to perform tasks required.

·Must be knowledgeable and follow Title 22 guidelines.

Delegation of Authority

Program Counselor is delegated the administrative authority, responsibility, and accountability necessary to carry out your assigned duties.

Job Functions

Administrative Functions

·Conduct comprehensive assessment of client needs.

·Develop mental health care plan for clients.

·Assist clients and clinicians with weekly psych/clinic visits and document progress.

·Assist clients and develop individualized service plans with specific goals and objectives.

·Monthly and PRN documentation in patient record.

·Communicate with Program Director regarding program needs, client needs and to all other department to ensure clients plan of care is being met.

·Assures and documents group attendance for each participant for assigned caseload.

·Facilitates group counseling per program schedule and individual counseling as needed.

·Provides input in weekly rounds and quarterly meetings.

·Assist clients in reaching their optimal level of psychosocial functioning through resolving negative patterns, prevention, rehabilitation and improving quality of life.

·Professional growth and behavior.

·Attendance at all weekly mandatory staff and in-service meetings.

·Performance of assigned daily duties.

·Monitoring patient safety.

·Crisis intervention and management.

·Non-Violent Crisis Management.

·Carrying out interventions specified on the patient care plan.

·Treating all patients with dignity and respect.

·Performance of other duties as requested by supervisor.

Other Responsibilities

·Open and monitor activity rooms.

·Respond to any request for emergency codes.

·Assist, if needed, in the dining room duties.

·Assist CNAs with monitoring hallways and rooms for resident safety.

·Assist nursing staff if needed in emergency situations with transportation of any resident outside the facility.

Staff Development

·Attend and participate in all mandatory in-service educational classes and on-the-job training programs.

·Duties and responsibilities assigned for the safe and continued operations of the facility.

Safety and Sanitation

·Reports all unsafe/hazardous conditions, defective equipment, etc., to your Supervisor immediately.

Equipment and Supply Functions

·Reports equipment malfunctions or breakdowns to your Supervisor as soon as possible.

·Ensures supplies have been replenished in work areas as necessary.

·Assures that work/assignment areas are clean and records, files, etc., are properly stored leaving such areas on breaks, end of work day, etc.

·Follow Title 22Regulations

Working Conditions

·Is subject to frequent interruptions.

·May be subject to working overtime hours, weekends and holidays, and other shifts/positions as necessary.

·Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.)

·Attends and participates in continuing educational programs.

·Is subject to injury from falls, burns from equipment, odors, etc., throughout the day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.

·Is subject to exposure to infectious waste, diseases, conditions, ect., including TB and the AIDS and Hepatitis B Viruses.

·Communicates with Rehab, Nursing and other department personnel.

·Is subject to hostile and emotionally upset residents, family members, personnel, visitors, etc.

·Is involved with residents, family members, personnel, visitors, government agencies and personnel, etc., under all conditions and circumstances.

·May be subject to the handling of and exposure to hazardous chemicals.

Specific Requirements

·Must be knowledgeable with PCC and its components.

·Must be able to read, write, speak and understand the English language.

·Must possess the ability to make independent decisions when circumstances warrant such action.

·Most possess the ability to deal tactfully with personnel, residents, visitors and the general public.

·Must possess the ability to work harmoniously with other personnel.

·Must possess the ability to minimize waste of supplies, misuse of equipment, etc.

·Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.

·Be able to follow written and oral instructions.

·Must maintain confidentiality.

·Must have good organizational and problem solving abilities.

·Be knowledgeable in micro-computers, data input and output, PCC, Microsoft office

·Must not pose a direct threat to the health or safety of other individuals in the workplace.
