Job Title: Care Management Director

Location: Glendale, California 91206

Shift : 7:30 A.M.–4:30 P.M.

Responsibilities:

Lead and oversee care management programs.

Ensure compliance with regulations and best practices.

Foster interdisciplinary collaboration for patient care.

Develop efficient care management protocols.

Analyze data for performance improvement.

Qualifications:

RN, MD, DO, or LCSW licensed in California.

BLS and Case Manager Certification.

Experience in care management leadership.

Strong communication and problem-solving skills.

Pay: $100.00 - $115.00 per hour

Expected hours: 40 per week
