As we continue to grow, we're seeking passionate Targeted Case Managers (TCMs) to join our dedicated team.

As a Targeted Case Manager, you'll play a crucial role in linking our clients with the resources and support they need to thrive in their communities. You'll work closely with a caseload of 20-25 clients, offering full-time linkage, referral, coordination, and monitoring services. Your work will prevent the need for higher levels of care by keeping our clients engaged in their communities, supported in their homes, and connected to essential services.
