Agency On Aging of South Central CT is Hiring a Care Manager Near North Haven, CT

Attention Care Managers

SUMMARY
:
For over forty-five years, the Agency on Aging of South-Central Connecticut (AOASCC) has been helping people age in their homes and remain engaged with their communities. We serve older adults, people with disabilities, and caregivers through various programs to provide care management, social services, assistance with government benefits, volunteer opportunities, and advocacy. We continue to grow and develop new programs to reach more community members who need our support.

GENERAL JOB DESCRIPTION:
As a Care Manager, you will work Monday through Friday, traveling to the client's home and completing assessments of the client's health, functional, and cognitive status to determine the need for continued home care services.
  • Primary responsibility to complete all the necessary annual assessments and respective paperwork to continue services the client may be eligible for and need.
  • Educate the client regarding available resources and program services.
  • Work with the client and/or the client's support system to complete all required applications for supportive services and programs.
  • Monitor with the client and/or the client's support system to evaluate the effectiveness of the plan of care, adjusting that plan as needed.
  • Complete all required documentation on time under the guidelines of the Agency and its State contract.
  • Prepare any required or requested reports.
  • Meet critical deadlines established by the funding source's internal procedures and ensure as little delay as possible in getting clients the needed services.

Position Requirements:
  • Applicants must have excellent interpersonal and organizational skills and experience with community resources and documentation requirements for state-funded programs.
  • Bachelor's (B.S.) degree in health, social work, gerontology, nursing, or related field plus a minimum of one year of experience working with older adults or those with disabilities.
  • Bilingual a plus
  • Applicants must have excellent clinical, interpersonal, and organizational skills and experience with community resources and documentation requirements for state-funded programs.
