Job Summary:

Provides family with supportive services, including prenatal and postpartum home visits, parent education in the areas of prenatal and postnatal care, breastfeeding, parent-infant attachment, child development and related topics. Identifies patient strengths, needs, barriers and risks. Facilitates referrals to community services and resources, as needed.

Job Requirements:

Education and Work Experience:

  • Bachelor’s Degree in child development, social work, psychology, public health, or equivalent (some certifications will require Bachelor's): Required
  • Master's Degree: Preferred
  • Maternal and child home visitation experience: Preferred

Licenses/Certifications:

  • Lactation educator, lactation counselor, or International Board Certified Lactation Consult (IBCLC) within six months of hire: Required

Essential Functions:

  • Collaborates with Welcome Baby program nurses, parent coaches, supervisors and other staff to provide a continuum of services. Serves as an interdisciplinary team member to help families ensure the health, safety and security of the new mother and baby. Develops trust and rapport with participants and provides support using an empathic and patient-centered approach. Adheres to program guidelines, policies and protocols and actively works toward achieving program goals. Conducts prenatal and post-partum visits in patient homes or over the phone, in accordance with the Welcome Baby program model.
  • Effectively conducts required patient assessments, using standardized tools for maternal depression, parent-infant attachment, social support, developmental milestones, home safety and non-standardized patient assessments. Identifies participant strengths, needs, barriers, and risks. Refers families to community services and resources, as needed. Provides education to parents about maternal and infant health, breastfeeding, parenting, attachment, home safety, family planning, child development and related topics using a participant-centered approach. Distributes and review the First Five Kit for New Parents with families. Observes, evaluates and provides consultation on breastfeeding. Observes parent-infant attachment and infant behavior.
  • Helps families establish a medical home, follows-up on well-child visits and identifies health insurance coverage, when necessary. Refers families to intensive home visitation services, as needed. Attends and participates in staff meetings, including bi-monthly group reflective supervision and weekly individual reflective supervision with clinical or team supervisors. Completes required paperwork for each patient in a timely, complete and organized manner, which includes clear and well written progress notes. Completes required individualized data entry in a timely manner.
  • Performs other job-related duties as assigned.
